What to Bring

To ensure your check-in process is as smooth and quick as possible, please remember to bring the following items to your scheduled appointment or walk-in visit.

Valid Photo ID

Driver's license, state ID, or passport.

Insurance Card

Primary and secondary (if applicable).

Current Medications

A list of all current prescriptions, over-the-counter medications, and supplements.

Patient Forms

Administrative Day (Wednesday)

Phone and portal messages are monitored; no patient appointments are scheduled.

Walk-In Window

For same-day urgent needs, walk-in slots are reserved Monday, Tuesday, Thursday, and Friday from 10:00 AM. Available on all clinical days (subject to availability).

Emergency Guidelines

Not Sure If You Need Us or the ER?

If you're experiencing a medical emergency (chest pain, difficulty breathing, severe bleeding, or loss of consciousness), call 911 immediately or go to your nearest emergency room.

For non-life-threatening acute illnesses (like fevers, minor lacerations, sprains, or sudden rashes), our clinic is equipped to handle your needs safely and more affordably than an ER visit.
